CAPE CANAVERAL — Elements of the next U.S. commercial cargo-delivery trip to the International Space Station are coming together at Kennedy Space Center for the planned March 10 liftoff.
Orbital ATK's Cygnus resupply ship, comprised of a pressurized cargo module that will carry 7,700 pounds of provisions, equipment and science experiments and a service module with thrusters, avionics and solar arrays, will be launched into orbit aboard a United Launch Alliance Atlas 5 rocket.
The cargo hold arrived last Friday, Jan. 15 at the Space Station Processing Facility and the propulsion tug arrived there yesterday, Jan. 21. The cylindrical cargo module was built by Thales Alenia Space in Italy and the service module was made by Orbital ATK in Virginia.
The initial cargo-loading will be performed at the SSPF before the two pieces are mated together.
The Enhanced Cygnus then moves next month to KSC's Payload Hazardous Servicing Facility for fueling, the final stowage of supplies and closure of the hatch for flight. ULA technicians then take over to encapsulate the craft in the rocket's nose cone.
Liftoff on March 10 is targeted for approximately 3:08 a.m. EST at the opening of a 30-minute launch window. Rendezvous with the station occurs three days later for grapple by the robotic arm.
This will be the second Atlas launch of a Cygnus in a 100-day span, lofting a combined cargo mass of over 15,000 pounds to the station.

NASA is investigating the source of black mold that contaminated cargo bags bound for the International Space Station, delaying the next launch of supplies from Florida's Space Coast.
An unmanned Orbital ATK Cygnus craft, which had been scheduled to launch March 10 atop a United Launch Alliance Atlas V rocket, now is targeting a March 22 liftoff from Cape Canaveral Air Force Station, according to ULA.
NASA said microbial analysis during a routine inspections at Kennedy Space Center found evidence of common black mold on two bags. The space agency decided to disinfect every bag, which required removing cargo already packed in the Cygnus.
It's the first time black mold has caused a problem during ISS cargo processing.
"Micro-organisms are everywhere and this type of micro-organism is not uncommon," said Dan Huot, a spokesman at NASA's Johnson Space Center, home of the ISS program. "NASA inspects, samples and disinfects cargo bags to ensure a safe environment for the crew members."
Molds can cause stuffiness, eye irritation, wheezing and skin irritation in people sensitive to them, according to the Centers for Disease Control and Prevention, and more severe reactions like fever and shortness of breath for those with serious allergies.
Earlier inspections had determined that the Cygnus vehicle's pressurized module was clean. The module arrived at KSC's Space Station Processing Facility last month after shipping across the Atlantic Ocean from manufacturer Thales Alenia Space's base in Italy.
Lockheed Martin is responsible for stowing ISS cargo under NASA's Cargo Mission Contract.
The upcoming resupply mission, labeled OA-6, is Orbital ATK's fifth under a NASA contract worth up to $3.1 billion. NASA recently awarded Orbital ATK one of three follow-on Commercial Resupply Services contracts, ensuring that the Cygnus will visit the ISS through 2024.
The mission is the second of two currently planned from Cape Canaveral launching a Cygnus on the Atlas V, following a successful first flight together in December that delivered about 7,700 pounds of cargo to the ISS.
Orbital ATK this summer aims to resume launches from Virginia of a redesigned Antares rocket, which is returning to flight after a failure in October 2014.
SpaceX is NASA's other commercial cargo provider over the next several years. The timing of its next launch of a Dragon capsule from Cape Canaveral — the first since a Falcon 9 rocket failure last June — is still to be determined.

NASA delays space station cargo run due to mold on packing bags

NASA's next cargo run to the International Space Station will be delayed for at least two weeks after black mold was found in two fabric bags used for packing clothing, food and other supplies, the U.S. space agency said on Wednesday.

The source of the mold, a common fungal growth in humid climates like Florida's, is under investigation by NASA and Lockheed Martin, which prepares NASA cargo for launch aboard two commercial carriers, Orbital ATK and privately owned SpaceX.

An Orbital Cygnus cargo ship was more than halfway packed for the launch, scheduled for March 10, when the mold was found during routine inspections and microbial sampling, NASA spokesman Daniel Huot said.

NASA and Lockheed decided to unpack the cargo, disinfect all the bags and then repack everything aboard the capsule, delaying launch.

The Cygnus' liftoff aboard a United Launch Alliance Atlas 5 rocket is now scheduled for late March.

Had the mold reached the station, the crew aboard would not have been in any danger, but NASA decided to clean the bags as a precaution, Huot added.

NASA is still assessing what, if any, impact delaying the Cygnus launch will have on the next cargo run planned by SpaceX. SpaceX has not delivered supplies to the space station since April 2015. Its last shipment was destroyed during a June 2015 launch accident.

Kennedy Space Center teams wielding hydrogen peroxide and wet wipes have averted the threat of a black mold infestation aboard the International Space Station.
The cleaning crews disinfected bags holding thousands of pounds of cargo that is expected to launch to the ISS next month inside an Orbital ATK Cygnus spacecraft, a precaution taken after tests found common black mold on two of the bags.
Investigation continues into the mold's source and how to prevent another contamination event, but NASA has confirmed that KSC and Florida humidity are not to blame for this one, which was the first of its kind.
That's because the microbial sampling, a standard pre-launch procedure, was performed before the cargo shipped from Houston to KSC. Test results did not become available, however, until after the cargo had been packed in the Cygnus.
"The contamination was identified from sampling in Houston, not Florida," said Dan Huot, a spokesman at NASA's Johnson Space Center.
The issue pushed back launch of the Cygnus by an Atlas V rocket from Cape Canaveral Air Force Station from March 10 to the current target date of March 22.
After the tests flagged the contamination, all cargo bags were removed from the Cygnus.
Personnel from Lockheed Martin, the contractor responsible for handling station cargo, applied enough hydrogen peroxide to penetrate the bags' woven fabric, then let them dry out. Handling equipment, tables and shelves also were cleaned.
Orbital ATK wiped down the Cygnus' interior with wet, lint-free wipes before the cargo was loaded back inside.
NASA says another batch of cargo on its way to Florida has been confirmed to be clean. The cargo is kept in a controlled environment throughout its travels, not exposed to outside humidity.
NASA did not confirm the total amount of cargo being launched, but the last Cygnus flight in December hauled roughly 7,700 pounds to orbit. That spacecraft, now packed with trash, is scheduled to depart the station Friday morning on its way to burning up in the atmosphere.
The next Cygnus mission will be Orbital ATK's fifth of 10 currently planned under a NASA Commercial Resupply Services contract worth more than $2 billion. One Cygnus did not reach orbit due to an Antares rocket failure in 2014.

CAPE CANAVERAL — Bound for the International Space Station with a max load of fresh supplies, a commercially-operated cargo ship was mounted atop its booster rocket today for liftoff from Cape Canaveral on March 22.
The Cygnus, produced by Orbital ATK, is filled with nearly four tons of food, equipment and science hardware for the station and its residents.
The craft will be lofted into orbit by a United Launch Alliance Atlas 5 rocket for a three-day trek to the station. Liftoff on March 22 will be possible during a 30-minute window opening at 11:05:48 p.m. EDT (0305:48 GMT).
"This will be the second launch for the Atlas to support the International Space Station program. We are very excited to support this particular mission," said Si Song, ULA's Atlas 5 spacecraft integration lead.
"We will launch at T-0 and about 21 minutes after launch we will separate the spacecraft."
In preparation for the flight, Cygnus was moved from its fueling depot at Kennedy Space Center's Payload Hazardous Servicing Facility to the Atlas rocket's assembly building at the Cape's Complex 41 early this morning.
A heavy-duty crane lifted the vessel, already shrouded in the rocket's 14-foot-diameter, 45-foot-long aluminum nose cone, into the Vertical Integration Facility for mating to the Centaur upper stage.
The finished product is a rocket that stands 194 feet tall. A mobile launching platform will roll the vehicle out to the pad next Monday.
ULA CEO Tory Bruno conducted his President's Mission Readiness Review last week that examined hardware pedigrees, technical issues on that bird and all sibling hardware, quality data and independent review, he tweeted.

Cygnus is headed to the station as part of NASA's Commercial Resupply Services program that hired Orbital ATK to carry cargo to the outpost from U.S. soil after retirement of the space shuttles.
This is Orbital ATK's sixth Cygnus built, the fifth operational mission to launch and the second in three months.
"We had a very successful OA-4 mission. We launched that in December and just de-orbited that and here we are, ready to launch the next one," said Dan Tani, former space shuttle and station astronaut and now the senior director for mission and cargo operations at Orbital ATK.
The cylindrical freighter is carrying:
-Crew supplies: 2,511 pounds
 -Vehicle hardware: 2,443 pounds
 -Science utilization: 1,713 pounds
 -EVA gear: 346 pounds
 -Computer resources: 216 pounds
The hatch on Cygnus was closed for flight on March 6.
"Once this Cygnus gets encapsulated (March 9), we are pretty much hands off our vehicle until we make contact with it in orbit and command the on-orbit initialization," Tani said.
The Cygnus will have three favorable launch opportunities — March 22, 23 and 24 — before facing a series of scheduling conflicts with a Russian Progress and SpaceX Dragon slated to launch at the end of March and early April for the International Space Station.
"If we launch on the 22nd or 23rd (of March), our first two launch opportunities, both of those days will get us to the ISS on the 26th. If we go to the 24th, we'll get there on the 28th. Then things get difficult on the space station," Tani said.
"We've been in conversation with them about what our options are and how they can be a little flexible. Of course it is in their best interest to get us docked and unloaded. So we are looking at 22nd, 23rd, 24th to get off, then we'll cross that bridge if we get to it."
The Atlas 5 rocket's remarkable string of 26 consecutive missions to launch on the first attempt after being fueled was snapped in December. The Cygnus deployment encountered three straight scrubs due to foul weather conditions.

Space-based manufacturing
In addition to transporting food and supplies for the astronauts aboard the International Space Station, Cygnus is transporting what is known as an "additive manufacturing facility," or AMF — a system using 3-D printing technology to allow the station crew to make needed parts and tools.
"The ability to manufacture on the ISS enables on-demand repair and production capability, as well as essential research for manufacturing on long-term missions," NASA said in its summary of the project. "AMF allows for immediate repair of essential components, upgrades of existing hardware, installation of new hardware that is manufactured, and the manufacturing capability to support commercial interests on the ISS."
Additive manufacturing is the process of efficiently building a part layer-by-layer, leading to reduced cost, mass, labor and production time.
"The ISS crew would be able to utilize the AMF to perform station maintenance, build tools, and repair sections of the station in case of an emergency," NASA said.
NASA said AMF will be a "permanent manufacturing facility" on the space station that can be upgraded in the future to add new functions and manufacturing methods.
Developed by California-based Made in Space, the AMF equipment is about twice the size as a 3-D printer previously delivered to the space station.
Made in Space's co-founder and chief technology officer, Jason Dunn, is a 2009 graduate of the University of Central Florida.

New approach to meteor research
One of the major new experiments coming to the space station involves meteor research that NASA says will be "important to our understanding of how the planets developed."
NASA said the Meteor Composition Determination experiment will mark the first space-based observations of the chemical composition of meteors entering Earth's atmosphere.
"Meteors are relatively rare, and are difficult to monitor from the ground because of the interference created by Earth's atmosphere," NASA said in its summary of the experiment. "The meteor investigation takes high-resolution video and images of the atmosphere, and uses a software program to search for bright spots, which can later be analyzed on the ground."
The experiment — which will be underway for two years, primarily during the peak periods of major meteor showers — will record spectral measurements which can reveal meteors' chemical makeup.
The experiment was developed by Chiba Institute of Technology in Narashino, Japan.

CAPE CANAVERAL — Working via remote control this spring, scientists will spark a fire aboard the unmanned Cygnus cargo ship that launches next Tuesday to study how the deliberate flames spread in weightlessness.
Aptly named the Spacecraft Fire Experiment, or SAFFIRE, the blaze will be contained within an instrumented research box — measuring 35 by 52 inches (3 by 4.4 feet) — that is strapped into the commercial freighter for the first-of-its-kind test.
The Cygnus, made by Orbital ATK, will be launched from Cape Canaveral on Tuesday night at 11:05 p.m. EDT (0305 GMT) atop a United Launch Alliance Atlas 5 rocket for a two-month spaceflight.
The craft is carrying over 7,000 pounds of food, goods and science gear to the International Space Station. It should arrive at the outpost next Saturday for a 55-day stay.
The SAFFIRE experiment will remain stashed inside Cygnus and then conducted after the supply ship departs the space station in May, with a load of garbage, and before the vessel re-enters the atmosphere 8 days later.
"The SAFFIRE experiment will ignite some sample material and the primary objective is to film the progression of the flame — a fire-time ever in such a large scale," said Dan Tani, former space shuttle and station astronaut and now the senior director for mission and cargo operations at Orbital ATK.
"We do combustion experiments on the space station, but the constraints on that experiment are 'don't kill the crew.' So all the safety constraints require all the experiments to be very small samples. Here we are, able to do a large sample flame propagation experiment."
Being able to light space fires for meaningful research has stymied scientists for decades due to the hazards involved in conducting tests large enough in a manned spacecraft. Cygnus freighters, however, offer ideal platforms to carry out tests after the cargo-delivery missions are completed and before re-entering the atmosphere for disposal.

The research is aimed at better understanding how fires in the cabin atmosphere will behave in weightlessness, quantify the risk, develop fire safety for future spacecraft and define the right types of fire detection, suppression and protection for astronauts trying to fight a blaze.
"The purpose for SAFFIRE is to address fire safety on exploration vehicles and habitats," said Gary Ruff, project manager and co-investigator for SAFFIRE at NASA's Glenn Research Center in Cleveland.
"The specific goals of the SAFFIRE experiments are to investigate the spread of a large-scale fire in microgravity, essentially trying to answer the questions of how large does a fire get and how rapidly does it spread, or how long does it take to get to the point of being really hazardous to the crew."
NASA intends to run SAFFIRE experiments on three consecutive Cygnus spacecraft launching through the end of this year. The SAFFIRE 1 and 3 tests will use single samples 15.7 inches wide by 37 inches tall to watch the development and spread of a large-scale low-gravity fire. Scientists want to know if there is a limiting flame size and to quantify the size and growth rate of flames over large surfaces.
"SAFFIRE is a box with a wind tunnel in it, a flow duct, that contains the sample that will be burned," said Ruff.
With two cameras poised to capture the fire, a hot wire along the upstream edge of the fiberglass-cotton fabric sample will trigger the burn that should last at least 15 or 20 minutes.
On SAFFIRE 2 later this summer, nine smaller samples, each measuring 2 inches wide x 10 inches long, will be burned sequentially to investigate the in-space "maximum oxygen concentration" flammability limits to determine if the flame propagates or self-extinguishes.
Prior space combustion experiments have been limited to samples no larger than 4 inches in length and width.

SAFFIRE instruments will measure oxygen, carbon dioxide, heat, pressure and flame growth, and two video cameras provide views of the flame. After the fire is started, the entire burn of each sample is recorded, then the data is compressed and downlinked to Earth. Cygnus will remain in orbit for several days to beam all the data to the ground.
"We will be evaluating the data...and getting ready for SAFFIRE 2 that will launch at the end of June," Ruff said.

NASA Glenn is leading an experiment that seeks to understand how fire spreads in a microgravity environment. Saffire I, II, and III will launch separately in 2016 aboard resupply missions to the ISS. But they will not be unloaded and after the Orbital/ATK Cygnus pulls far away fr om the space station, the experiments will begin. This animation explains how it all works.

Understanding how fire spreads in a microgravity environment is critical to the safety of astronauts who live and work in space. And while NASA has conducted studies aboard the space shuttle and International Space Station, risks to the crew have forced these experiments to be limited in size and scope.

Now a new experiment, designed, built and managed at NASA's Glenn Research Center, will ignite an understanding of microgravity fire on a much larger scale. The Spacecraft Fire Experiment, known as Saffire, is a series of experiments to be launched on three different flights beginning in March.

"A spacecraft fire is one of the greatest crew safety concerns for NASA and the international space exploration community," says Gary Ruff, Saffire project manager.

Saffire will involve far larger flames than previous experiments and will investigate the way fire spreads on a variety of combustible materials. Because the experiments will be conducted away from the space station, there is no risk to the astronauts aboard.

Each Saffire experiment will be remotely operated inside a 3 x 5 foot module, split into two compartments. One side of the module is an avionics bay that contains sensors, high definition video cameras and signal processing equipment. The other side contains the hardware required to ignite a large flame and burn the fabrics and materials inside.

When the experiments begin, Saffire I and III will burn one large 16 by 37-inch piece of SIBAL cloth, which is a blend of fiberglass and cotton. This material has been studied in previous microgravity combustion experiments, although at a much smaller size. The SIBAL cloth will be burned from the bottom to see how the flame spreads. If the flame extinguishes itself, scientists will light it at the top and see what happens as the flame moves opposite to the airflow.

Saffire II, scheduled to launch in June from Wallops Flight Facility in Virginia, will ignite a mix of nine different samples of materials used routinely on the space station including flame retardant fabrics used for astronaut clothing, station Plexiglas window samples with edge variations and structures used for storage containers and silicone composites. Each sample is two by 11 inches, the size sample NASA uses to screen materials on Earth before they are used on a spacecraft.

"Saffire seeks to answer two questions," says David Urban, principal investigator. "Will an upward spreading flame continue to grow or will microgravity lim it the size? Secondly, what fabrics and materials will catch fire and how will they burn?"

The Saffire I payload will hitch a ride on a resupply mission to the space station in an Orbital ATK Cygnus cargo vehicle from Kennedy Space Center in Florida. When Cygnus arrives, astronauts will unload their supplies, but Saffire will remain on board Cygnus.

"Within the first day after Cygnus pulls away from the space station, we will begin the experiment, which will run autonomously once the RUN command is sent," says Steven Sinacore, deputy project manager. "It will only take a few hours to run the experiments, but Cygnus will remain in space for seven days to ensure complete data transmission back to the Saffire operations team on the ground." Eventually, Saffire, along with Cygnus, will be destroyed upon reentry into Earth's atmosphere.

Concepts for additional Saffire missions- IV, V, VI are in development to focus more on flame spread, smoke propagation, detection and suppression of fire.

As NASA continues to send astronauts to the space station and continues the path toward a human mission to Mars, improving understanding of the structure of spacecraft fires is critical. "Saffire is all about gaining a better understanding of how fire behaves in space so NASA can develop better materials, technologies and procedures to reduce crew risk and increase space flight safety," says Ruff.

Saffire's flight systems were sponsored by NASA's Advanced Exploration Systems (AES) Program. AES develops foundational technologies and high-priority capabilities that are the building blocks for future human space missions. AES technologies are developed using in-house capabilities and NASA's workforce while fostering public-private partnerships.

The first Saffire experiment will assess a large-scale microgravity fire using sample material that is approximately 16 inches wide and 37 inches long. Two subsequent flight experiments in 2016 consist of Saffire-II, which will assess oxygen flammability limits using samples that are 2 inches wide and 12 inches long; and Saffire-III, which will assess a second large-scale microgravity fire. Each module will be flown aboard an Orbital ATK Cygnus cargo vehicle during a resupply mission to the space station.

The Spacecraft Fire Safety Demonstration Project that developed the Saffire flight systems is sponsored by the Advanced Exploration Systems Division in NASA's Human Exploration and Operations Mission Directorate. The three Saffire units were built at Glenn using components fabricated in manufacturing facilities at Glenn, NASA's Johnson Space Center in Houston, and White Sands Test Facility in Las Cruces, N.M. Three additional experiment modules will be designed and built by engineers at Glenn and launched in Orbital ATK cargo vehicles beginning in 2018.

After the fire event, launching a series of tiny cubesats from Cygnus will be the next order of business.

"We will unberth from the space station, perform the SAFFIRE experiment, then we will phase away safely from the station and get into an exact orbit that the trajectory folks at NASA feel is an appropriate distance and angle away. Then we will deploy the nanosats," Tani explained.

Upon the plunge into the atmosphere over the South Pacific, one final experiment will be conducted by the Cygnus, called the Re-entry Breakup Recorder.

"It is a series of sensors and a re-entry-survivable collector, a big sphere. The crew will install that into the Cygnus. Once we depart, we will do a series of maneuvers specifically designed for REBR to give it accelerations so it understands it needs to turn itself on.

"As we do our destructive re-entry into the atmosphere, the REBR will monitor pressures and temperatures and accelerations. The survivable unit will preserve that data, and after it splashes into the ocean it will send its data via commercial satellite telephone," Tani said.

By Steven Siceloff,
 NASA's Kennedy Space Center, Florida

A new 3D printer and research projects examining everything from adhesive technologies to the behavior of large fires in space are packed inside an Orbital ATK Cygnus spacecraft for launch Tuesday, March 22, at 11:05 p.m. EDT from Cape Canaveral Air Force Station in Florida. The launch window extends for 30 minutes.
Launching atop a United Launch Alliance Atlas V rocket, the Cygnus, which carries no crew, will steer itself to the station during the course of three days. Astronauts and ground controllers will use the station's robotic arm to grapple Cygnus and connect it to the Earth-facing port of the Harmony module. Cygnus will stay connected to the Earth-facing laboratory for about two months before being released to burn up in Earth's atmosphere.
Much of the science will be conducted by astronauts aboard the International Space Station as they continue landmark research above Earth for the benefit of those on the Earth and future astronauts making the journey to Mars. Other science, such as the fire research and reentry data collection, will be conducted at the end of the mission and only after Cygnus drops off its materials and is flying on its own far from the station.
Named the S.S. Rick Husband in tribute to the astronaut who commanded the STS-107 mission which was lost Feb. 1, 2003, this will be the second flight of an enhanced version of Cygnus which first flew in December on a successful return to the flight for the company. Able to carry about 25 percent more volume than its predecessor, the enhanced models also feature more efficient solar arrays and other upgraded systems.
While docked to the station, Cygnus will be unloaded by astronauts who will set up the experiments and stow the fresh supplies. Altogether, the mission's cargo manifest totals more than 3 ½ tons, including experiments by government and private researchers. Two expeditions – 47 and 48 – will conduct the research in NASA's continuing drive to unlock the secrets of long-duration space exploration.
"It's like Christmas when a supply craft arrives," said Orbital ATK's Dan Tani, a former shuttle and station astronaut who is now senior director of mission cargo and operations. "It's always fun to watch another vehicle approach and then it's like opening a box of goodies and finding some stuff you've been wanting and some surprises you didn't know about."
This Cygnus will carry more to the station than any of the previous five missions, Tani said.
A few of the scientific highlights:
- Gecko Gripper, testing a mechanism similar to the tiny hairs on geckos' feet that lets them stick to surfaces using an adhesive that doesn't wear off,
- Strata-1, designed to evaluate how soil on small, airless bodies such as asteroids behaves in microgravity.
- Meteor, an instrument to evaluate from space the chemical composition of meteors entering Earth's atmosphere. The instrument is being re-flown following its loss on earlier supply missions.
- Saffire, which will set a large fire inside the Cygnus in an unprecedented study to see how large fires behave in space. The research is vital to selecting systems and designing procedures future crews of long-duration missions can use for fighting fires.
- Cygnus is carrying more than two dozen nanosatellites that will be ejected from either the spacecraft or the station at various times during the mission to evaluate a range of technology and science including Earth observations.
The station residents depend on cargo missions from Earth to supply them with daily clothes, food, water and air, along with the equipment they need to work in orbit. For instance, this mission is carrying a spacesuit for the crew and high-pressure cylinders to recharge the station's air supply.
As the Cygnus approaches the end of its time connected to the station, astronauts will pack it with trash, spent experiments and other equipment no longer needed. It will all burn up as the spacecraft blazes through the atmosphere to end the flight with a safe impact in the Pacific Ocean.

"This would place the 1st stage burn time 5.4 seconds shorter than specified in the published launch timeline"
"These data points would suggest that Atlas V's first stage shut down too early which could be an explanation for Centaur extending its burn and making up for delta-v lost during first stage flight. An early shutdown of the Common Core Booster could be caused by a number of issues, the prime suspect first to be checked off the list would be a premature depletion of either LOX or Rocket Propellant 1 and/or issues related to the RD-180 engine."

ToryBruno ULA's President & CEO

Atlas first stage burned just under 6 seconds short. That left a delta-V deficit that Centaur needed to make up. Centaur made up that difference by burning about a minute long, delivering Cygnus into a precise orbit, far better that the accuracy requirement (as is usual). However, this consumed propellant, so Centaur's planned de-orbit burn was shorter. Consequently, Centaur impacted in the ocean a little further downrange than the original plan.
While this highlights the robustness of the system, we don't like anything happening that was not expected and are investigating this thoroughly. That's one of the reasons we have the mission success record we enjoy: Thoroughness. Every bit of data and every feature gets ground to dust by our engineers after every flight. (which is paid for on USAF missions by their ELC contract, BTW...)
In this case, because we consider it an anomaly, a very specific and structured set of investigation protocols is followed with a dedicated team.
We have a pretty good idea about the cause, but won't share until we are sure. That is another important discipline. We avoid jumping to the answer until the investigation is complete. That prevents us from narrowing in on a tempting answer too quickly before all the possibilities have been thoroughly investigated. By approaching investigations in this disciplined manner, you have the highest confidence of arriving at a complete, global, and permanent corrective action.
Over the years, I've found that its poor practice and bad luck to jump to the easy answer before doing the homework.
At present, I am not concerned about impacts to the manifest.

Centennial, Colo., (March 31, 2016) -- ULA successfully launched and deployed the OA-6 Cygnus spacecraft to its desired orbit on Tuesday, March 22. During the launch, the system experienced a premature first stage shutdown. The first stage cut-off occurred approximately 6 seconds early; however the Centaur automatically compensated by burning longer. The robustness of the system achieved mission success, delivering Cygnus to a precise orbit as planned. The ULA engineering team is continuing to review the data and has developed initial fault trees with our industry partners. The team has been successful in isolating the anomaly to the first stage fuel system and its associated components. Per standard processes when a flight anomaly such as this has been identified, ULA and our partners have formed a robust review team with full collaboration with major customers. The review will thoroughly assess all flight and operational data to determine root cause and identify appropriate corrective actions prior to future flights.

The Orbital ATK Cygnus cargo ship is seen after final approach to the International Space Station. The vehicle was captured at 6:51 a.m. EDT March 26 using the space station's Canadarm2 robotic arm by Expedition 47 Commander Tim Kopra. The unmanned cargo craft was then bolted to the Earth-facing port on the Unity module at 10:52 a.m. Orbital ATK's fifth cargo delivery flight under its Commercial Resupply Services contract delivered over 7,700 pounds of cargo and included equipment to support some 250 experiments during Expeditions 47 and 48.

After delivering almost 7,500 pounds of cargo to support dozens of science experiments from around the world, the Orbital ATK Cygnus cargo spacecraft is set to leave the International Space Station Tuesday, June 14. NASA Television will provide live coverage of Cygnus' departure beginning at 9 a.m. EDT.
Ground controllers will detach the Cygnus spacecraft, which arrived at the station March 26, from the Earth-facing side of the station's Unity module using the Canadarm2 robotic arm. Robotics controllers will maneuver Cygnus into place and Expedition 47 robotic arm operators Tim Kopra of NASA and Tim Peake of ESA (European Space Agency) will give the command for its 9:30 a.m. release.
Five hours after departure, the Saffire-I (https://www.nasa.gov/press-release/nasa-pursues-burning-desire-to-study-fire-safety-in-space) experiment will take place onboard the uncrewed cargo craft. Saffire-I provides a new way to study a realistic fire on a spacecraft. This hasn't been possible in the past because the risks for performing such studies on crewed spacecraft are too high. Instruments on the returning Cygnus will measure flame growth, oxygen use and more. Results could determine microgravity flammability limits for several spacecraft materials, help to validate NASA's material selection criteria, and help scientists understand how microgravity and limited oxygen affect flame size. The investigation is crucial for the safety of current and future space missions.
Cygnus also will release five LEMUR CubeSats (http://www.nasa.gov/mission_pages/station/research/experiments/2349.html) from an external deployer June 15, part of a remote sensing satellite constellation that provides global ship tracking and weather monitoring. The vehicle will remain in orbit until Wednesday, June 22, when its engines will fire twice, pushing it into Earth's atmosphere wh ere it will burn up over the Pacific Ocean. NASA TV will not provide a live broadcast of the Cygnus deorbit burn and re-entry.
Experiments delivered on Cygnus (http://www.nasa.gov/mission_pages/station/research/news/crs6_science) supported NASA and other research investigations during Expeditions 47 and 48, including studies in biology, biotechnology, physical science and Earth science -- research that impacts life on Earth, and also will help us on the journey to Mars. Investigations studied realistic fire scenarios on a space vehicle, enabled the first space-based observations of meteors entering Earth's atmosphere from space, explored how regolith behaves and moves in microgravity, tested a gecko-inspired adhesive gripping device (http://www.nasa.gov/jpl/gecko-grippers-moving-on-up) that can stick on command in the harsh environment of space, and added a new 3-D printer in microgravity.
The Cygnus resupply craft launched March 22 (http://www.nasa.gov/press-release/nasa-sends-fire-meteor-experiments-to-international-space-station-on-commercial-cargo) on a United Launch Alliance Atlas V rocket from Cape Canaveral Air Force Station, Florida, for the company's fifth NASA-contracted commercial resupply mission.

Space.com (http://www.space.com/33173-nasa-fire-in-space-experiment-saffire.html)

Status Report From: United Launch Alliance (http://www.ulalaunch.com/)
Posted: Wednesday, June 15, 2016

ULA successfully delivered the OA-6 Cygnus spacecraft to the International Space Station (ISS) on March 22. During the first stage burn, the Atlas V system experienced an anomalous propellant mixture ratio resulting in an early booster shutdown and degradation of first stage performance. The Atlas V's robust system design, flight software, vehicle margins and propellant reserves enabled the successful outcome for this mission.
The Centaur upper stage identified the first stage performance shortfall and compensated with an extended burn to deliver Cygnus to the precise orbit, well within the required accuracy.
Per standard processes when a flight data item such as this has been identified, the ULA engineering team, along with our engine supplier and several government customers, forms a robust review team. The review team assessed all flight and operational data to determine direct and root causes and implemented the appropriate corrective actions for future flights.
"Thanks to the robust vehicle system design of Atlas V, the OA-6 mission was delivered successfully to its intended orbit and Cygnus completed its' mission to the ISS," said Gary Wentz, ULA's vice president of Human and Commercial Systems. "ULA applied a rigorous anomaly review process to identify and implement all necessary corrective actions to ensure we continue to reliably deliver critical capabilities for our customers with 100 percent mission success."
An extensive review of all post-flight data observations determined that at approximately T+222 seconds, an unexpected shift in fuel pressure differential across the RD-180 Mixture Ratio Control Valve (MRCV) and a reduction in fuel flow to the combustion chamber caused an oxidizer-rich mixture of propellants and a reduction in first stage performance. The imbalanced propellant consumption rate resulted in depletion of the first stage oxidizer with significant fuel remaining at booster engine shutdown. The engine supplier has implemented a minor change to the MRCV assembly to ensure the anomaly does not occur on future flights.
Engine hot-fire testing, extensive component and assembly level testing and analyses have been performed to confirm the findings. Detailed inspections and minor hardware replacement on all engines will support the current launch manifest.
"We would like to thank our customers and supplier partners for their outstanding collaboration in the detailed review of this anomaly. We are honored to be entrusted with delivering America's most critical space assets to orbit and appreciate the outstanding team effort in delivering 100 percent mission success - one launch at a time," said Laura Maginnis, ULA's vice president of Custom Services. "'We remain on plan to launch all of our manifested 2016 missions within the year."
The final and corrective action reviews, as well as flight clearance for the Atlas V MUOS-5 mission occurred on June 2. The Atlas V MUOS-5 launch is scheduled for June 24 from Space Launch Complex-41 at Cape Canaveral Air Force Station, Fla.
With more than a century of combined heritage, United Launch Alliance is the nation's most experienced and reliable launch service provider. ULA has successfully delivered more than 100 satellites to orbit that provide critical capabilities for troops in the field, aid meteorologists in tracking severe weather, enable personal device-based GPS navigation and unlock the mysteries of our solar system.

OA-6 Mission Demonstrates Versatility and Value of Cygnus Spacecraft

"S.S. Rick Husband" Provided Record Shipment of Vital Cargo to International Space Station and Used as Technology Platform for In-Space Science Experiments

Dulles, Virginia 22 June 2016 – Orbital ATK, Inc. (NYSE: OA), a global leader in aerospace and defense technologies, today announced the successful completion of its second cargo delivery mission in 2016 to the International Space Station under NASA's Commercial Resupply Services-1 (CRS-1) contract. The mission, known as OA-6, marked the company's fifth CRS-1 mission and the first time its Cygnus™ has been used as an in-orbit research platform. Orbital ATK, which named the Cygnus spacecraft the "S.S. Rick Husband" in honor of the late NASA astronaut, carried approximately 7,900 pounds (3,600 kilograms) of science experiments and cargo to the space station. The spacecraft also executed on three post-departure mission objectives, including the Spacecraft Fire Experiment-I (Saffire-I), the deployment of CubeSat satellites from the NanoRacks CubeSat Deployer and the Reentry Breakup Recorder with Wireless Sensors (REBR-W) test.

The Cygnus spacecraft that carried out the OA-6 mission departed the International Space Station on June 14, completing an 81-day stay. Prior to its departure, the astronauts loaded the cargo module with approximately 4,087 pounds (1,854 kilograms) of items for disposal. Earlier today, at approximately 9:29 a.m. EDT, Cygnus performed a safe, destructive reentry into Earth's atmosphere over the Pacific Ocean east of New Zealand, marking the successful conclusion of the mission.

"Throughout the OA-6 mission, we were able to demonstrate the versatility and value of our Cygnus spacecraft, from its launch on an Atlas rocket, to the flawless delivery of the largest load of pressurized cargo in the CRS program's history, to its ability to serve as a platform for in-space research," said Frank Culbertson, President of Orbital ATK's Space Systems Group.

On March 22, Cygnus demonstrated its compatibility with multiple launch vehicles for the second time as it lifted off from Cape Canaveral Air Force Station, Florida aboard a United Launch Alliance Atlas V rocket. Four days later, Cygnus docked with the International Space Station, delivering vital supplies and science experiments to the astronauts on board. As it departed the space station, Cygnus then served as an in-orbit research platform, further displaying its robust capabilities.

The first experiment to take place onboard Cygnus was Saffire-I, a technology demonstration designed by NASA's Glenn Research Center. Saffire-I begins a series of tests to study the behavior of large scale fires in microgravity. Orbital ATK successfully sent the command to initiate the experiment, which intentionally lit a fire that occurred in the Cygnus pressurized cargo module. The experiment executed as intended and NASA's Glenn Research Center is currently evaluating all data and imagery gathered from the test. Saffire-I and future Saffire experiments directly support the development of technologies and materials that will make deep space exploration spacecraft safer for astronauts who live and work in space and as NASA prepares for long duration missions on the journey to Mars.

Following the Saffire-I experiment, Cygnus then successfully deployed four of five CubeSats via a NanoRacks CubeSat Deployer. The satellites are now in orbit conducting their individual missions.
The final experiment of the OA-6 mission took place during Cygnus' reentry into Earth's atmosphere. Astronauts installed the Reentry Breakup Recorder with Wireless Sensors (REBR-W) on Cygnus prior to its departure from the station. REBR-W, developed by The Aerospace Corporation, will allow for better understanding of spacecraft behavior during atmospheric reentry and breakup, leading to improved vehicle safety and design in the future. REBR-W recorded and transmitted Cygnus' safe, destructive reentry into Earth's atmosphere that marked the successful conclusion of the OA-6 mission.

Preparations are underway for the company's next CRS-1 mission, OA-5, which is targeted to launch later this summer from the Mid-Atlantic Regional Spaceport at NASA's Wallops Flight Facility in Virginia. Orbital ATK is scheduled to complete 10 CRS-1 missions to the International Space Station before transitioning to launch cargo logistics missions under the CRS-2 contract in 2019.

...
Crew members spent part of the week preparing for the June 14 departure of the Cygnus resupply ship, including activating a device that will provide insight into just how the ship breaks apart as it falls toward Earth.

NASA astronaut Jeff Williams installed and activated the Reentry Breakup Recorder with Wireless Sensors (REBR-W) into the Orbital ATK Cygnus spacecraft attached to the station. When the capsule re-enters Earth's atmosphere June 22, the device inside the vehicle will record data about when and how the craft breaks apart. This information can be used for reentry hazard prediction studies, reducing risks and improving planning for spacecraft that eventually will deorbit.

If a spacecraft's mission ends because of an accident, a broken part, low propellant, or failing components, NASA and the Department of Defense require that it re-enter over unpopulated areas -- such as the open ocean -- to prevent injury to people or damage to property. This sometimes means de-orbiting a spacecraft before it fails completely to ensure control over where it comes down. Data from the REBR-W device will provide new insight that could eliminate the need for deorbiting a spacecraft before it naturally falls to Earth. This would extend mission life and reduce cost and complexity while minimizing risk. The investigation may also lead to new ways for scientists to perform hypersonic flight testing, test new thermal protection materials, and study the uppermost layers of the atmosphere.

Cygnus OA6 Reentry Movie (29 MByte) (http://atv5.seti.org/cygnus/cygnus_video.avi)
June 22, 2016 - Success! Cygnus OA6 re-entered at 13:29 UT, June 22, creating a horizon-to-horizon meteor. Observers onboard a chartered aircraft for the Cygnus OA6 "S.S. Rick Husband" Re-entry Observation Campaign first detected the vehicle as a star-like object near the western horizon. Conditions were ideal, with a clear horizon and cloud cover well below the aircraft. The aicraft flew an observing leg several 100's of km north of the entry trajectory at an altitude of 43,000 ft. Mike Koop and Jim Albers managed to isolate the earlier interference and make our GPS and timing equipment correctly lock onto the gps network. A full moon lit the landscape. While slowly approaching the viewing area, Peter Jenniskens recorded how Cygnus became rapidly brighter, then more slowly so. It was imaged passing by the plane from the cockpit by Jim Albers and when it emerged on the oher side and in view of the high resolution imagers and spectrographs of the Red Team, led by Ron Dantowitz, it was a distinct orange in color and created a slew of fragments. There was no bright flare from a main disruption. Instead a number of smaller events gradually broke the vehicle into a cloud of debris. Ever changing spectroscopic signatures were detected that are expected to shed light on the sequence of how the vehicle breaks up in such a shallow reentry. The DIM instrument operated by Janna Feeley tracked the object and its wake until it slowed more, turned deep orange, and finally disappeared behind the eastern horizon. The final moments of Cygnus OA6 were captured in the still image by Eleanor Sansom shown above. Cygnus, thank you!
